Are you a Nurse Practitioner passionate about personalized, community-based care? Join a growing healthcare organization delivering compassionate, high-quality care to patients in their homes and in senior living settings. In this role, you will provide primary care services primarily through home visits and in senior care facilities, including assisted living and long-term care. You'll manage patient care plans, conduct post-hospital assessments, and collaborate with schedulers to optimize your visits.

How You'll Contribute

  • Conduct home-based and senior facility patient visits (approx. 85% home, 15% facility)
  • Perform primary care evaluations, follow-up, and care management
  • Diagnose, treat, and create care plans
  • Prescribe medications and order diagnostic tests as needed
  • Submit referrals and coordinate with schedulers
  • Educate patients and families on preventive and ongoing care
  • Monitor patient progress and update treatment plans accordingly
  • Document encounters in an EMR system (Charm Health or similar) within 24 hours
  • Consult with the Medical Director when necessary

Schedule & Coverage:

  • Flexible 3-day workweek; provider sets their own schedule
  • Day shifts typically 8:00 AM to 5:00 PM
  • No on-call requirements
  • Orientation: 2 weeks, plus up to 30 days for credentialing
  • Shadowing available upon request

Locations Covered:

  • Richmond, VA (within a 40-mile radius)
  • Fredericksburg, VA (within a 30-mile radius)
  • Tappahannock, VA / Northern Neck (within a 40-mile radius) — Note: Mileage reimbursement provided.

What Makes You a Great Fit

  • Active Nurse Practitioner license in the state of Virginia
  • Current DEA license
  • Active CAQH profile and number
  • Minimum of 1 year of relevant NP experience in ALF, LTC, Senior Care, Primary, or Family Care
  • Preferred: additional experience in Urgent Care, ER, Trauma, or Inpatient Acute Care
  • Proficient in using EMR systems
  • Malpractice insurance provided
