This contract Nurse Practitioner (NP) role is a vital addition to the clinical team at West Park LTC. As a Nurse Practitioner, you’ll help bridge gaps in healthcare delivery, prevent unnecessary hospital transfers, and lead the way in evidence-based care planning for geriatric and medically complex residents.

You’ll be a clinical leader and educator for staff, helping ensure every resident receives thoughtful, person-centered care.

How You'll Contribute

As the Nurse Practitioner, your core responsibilities will include:

  • Conducting comprehensive health assessments and medical histories

  • Diagnosing, treating, and managing acute and chronic conditions

  • Prescribing medications and managing polypharmacy

  • Participating in care conferences and communicating with families

  • Providing clinical leadership and support to the nursing and PSW teams

  • Educating staff on disease prevention, infection control, and best practices

  • Supporting hospital diversion initiatives and continuity of care

  • Collaborating with physicians, specialists, allied health, and administration

  • Participating in emergency response and on-call rotations (as agreed upon)

What Makes You a Great Fit

We’re looking for someone who brings both clinical excellence and a deep commitment to compassionate senior care. The ideal candidate will have:

  • A valid Nurse Practitioner license in Ontario and active CNO registration

  • An OHIP billing number and comfort operating as an independent contractor

  • 3+ years of NP experience (preferably in long-term care or geriatrics)

  • A background in complex care, dementia, and chronic disease management

  • A collaborative, educational mindset to guide team learning and development

  • Strong clinical judgement, communication, and family engagement skills

  • Flexibility to work full-time or part-time and openness to on-call support
