How You'll Contribute
Deliver high-quality housing stability services, ensuring members receive exceptional care navigation.
Conduct comprehensive assessments to identify members' housing needs and develop personalized care plans.
Provide guidance and support to members navigating housing instability, addressing barriers, and facilitating access to resources.
Collaborate with interdisciplinary teams to ensure coordinated care and support for members.
Utilize expertise in housing instability to inform program development and service delivery.
Maintain accurate documentation and data records in compliance with regulatory requirements and timelines.

What Makes You a Great Fit
Bachelor’s degree in social work, social services or related field.
Demonstrated expertise in housing instability, related social determinants of health or mental health.
Proven ability to collaborate effectively.
Capable of working in both office and remote environments.
