How You'll Contribute

  • Assist in managing daily clinical and administrative operations across the home health program
  • Provide leadership, coaching, scheduling guidance, and mentorship to clinical staff
  • Monitor care quality, timeliness of visits and documentation, and regulatory compliance
  • Support and participate in performance improvement and quality assurance initiatives (QAPI)
  • Collaborate with clinicians to ensure consistent, high-quality, patient-centered care plans and outcomes
  • Track metrics and escalate barriers to care delivery, staffing, and patient satisfaction
  • Partner with intake, scheduling, and field teams to optimize throughput and timely start-of-care
  • Ensure adherence to CMS Conditions of Participation and agency policies
  • Contribute to staff training, competency validation, and policy updates
  • Serve as a resource for problem-solving, case conferencing, and interdisciplinary coordination

What Makes You a Great Fit

  • Registered Nurse (RN) license in Texas preferred
  • 5+ years of home health or related healthcare experience
  • 2+ years of home health leadership or supervisory experience
  • Demonstrated knowledge of CMS regulations and home health standards of practice
