How You'll Contribute
- Assist in managing daily clinical and administrative operations across the home health program
- Provide leadership, coaching, scheduling guidance, and mentorship to clinical staff
- Monitor care quality, timeliness of visits and documentation, and regulatory compliance
- Support and participate in performance improvement and quality assurance initiatives (QAPI)
- Collaborate with clinicians to ensure consistent, high-quality, patient-centered care plans and outcomes
- Track metrics and escalate barriers to care delivery, staffing, and patient satisfaction
- Partner with intake, scheduling, and field teams to optimize throughput and timely start-of-care
- Ensure adherence to CMS Conditions of Participation and agency policies
- Contribute to staff training, competency validation, and policy updates
- Serve as a resource for problem-solving, case conferencing, and interdisciplinary coordination
What Makes You a Great Fit
- Registered Nurse (RN) license in Texas preferred
- 5+ years of home health or related healthcare experience
- 2+ years of home health leadership or supervisory experience
