How You'll Contribute

  • Support daily clinical and administrative operations across the home health agency
  • Provide leadership, coaching, scheduling guidance, and support to clinical staff (RNs, LVNs, therapists, aides)
  • Monitor and ensure care quality, timeliness of visits, and regulatory compliance (CMS/Home Health CoPs)
  • Participate in and help lead Performance Improvement (PI/QAPI) programs and data-driven initiatives
  • Collaborate on care planning, case conferencing, and discharge planning to ensure patient-centered outcomes
  • Track key quality metrics (timely care, functional outcomes, rehospitalizations) and implement corrective actions
  • Standardize and optimize workflows, documentation practices, and communication processes
  • Support survey readiness and respond to audits; maintain policy and procedure adherence
  • Foster a culture of accountability, continuous improvement, and high patient satisfaction
  • Partner with leadership on staffing, resource allocation, and growth initiatives

What Makes You a Great Fit

  • RN license in Texas preferred (active, in good standing)
  • 5+ years of home health or closely related healthcare experience
  • 2+ years of leadership/supervisory experience in home health
  • Demonstrated knowledge of CMS regulations, Home Health Conditions of Participation, and quality standards
  • Proven ability to coach and develop clinical staff while maintaining high performance and compliance
  • Strong communication, organizational, and problem-solving skills
  • Ability to manage multiple priorities, interpret quality data, and drive process improvement
  • Commitment to delivering consistent, high-quality, patient-centered care
  • Availability to work on-site/in-field as needed to support operations
